When you come into for an appointment for your tax return, please don't forget:

• All W-2s
• 1099: interest earned
• 1098: mortgage interest statement
• Name, address, social security number & amount paid to all daycare providers
• Dependents: full name, social security number & date of birth
• Personal property tax paid
